The Counseling & Health Services offers integrated primary health care, personal counseling, health education, and wellness programs to enrolled APSU students. Our goal is to promote academic and personal success by encouraging our students to be mindful and proactive and make informed decisions about their physical and mental wellness.

Appointments can be scheduled with Student Counseling Services by calling (931) 221-6162.

Student of Concern Form

The Student of Concern Form is intended to provide guidance in those instances where a university community member becomes aware of a person whose behavior is concerning, or whom you are concerned about. In particular, this report facilitates the reporting of questions or concerns about any student who may pose a potential threat to their own health and safety or to the health and safety of others.

 

 

 

Discrimination and Harassment Form   

Inquiries or complaints regarding the non-discrimination policies for members of the APSU community, including Title IX complaints, should be directed immediately to LaNeeça R. Williams, Chief of Institutional Culture and Title IX Coordinator or by filling out the confidential APSU Discrimination and Harassment Complaint Form.

 

 

 

Divisive Concepts Allegation Form

Pursuant to T.C.A. §49-7-1903, a student or employee who believes that the institution or an employee has engaged in prohibited activity may file a complaint using the Divisive Concepts Alleged Violation Report Form or by sending an email to divisiveconcepts@apsu.edu.

Incident/Complaint Form

Any member of the APSU University community (staff, faculty, or student) has the right to file a disciplinary complaint against a student or a student organization by contacting the office of Community Care & Standards or completing the Incident/Complaint Form below.
